A gentle, step‑by‑step guide to help you feel prepared, comfortable, and confident before your first visit.

What to Expect

Your first visit is relaxed, unhurried, and focused on understanding your health story. Most new patients say it feels more like a conversation than a doctor’s appointment.


During your visit, we will:

  • Talk through your main concerns, symptoms, and goals
  • Review your health history and any medications or supplements
  • Discuss simple next steps you can take right away
  • Decide together whether ongoing support or a membership makes sense for you


There is no pressure to join a membership. Many patients choose to stay as non‑members and simply pay per visit.

How to Prepare

A little preparation helps your visit go smoothly.


Complete:

  • Your intake forms (sent to you electronically after scheduling)
  • Your intake form will request that you provide a list of your current medications and supplements

You can email your T Wellness healthcare provider with:

  • Any recent labs or imaging you’d like them to review


If you have any questions or concerns—nothing is too small. You may include these in your electronic intake, email them to your provider, or prepare those questions to be discussed at your appointment.


If you’re unsure about any additional preparation, we can gather information together at your appointment.

Membership Overview

Membership is optional. It’s designed for patients who want predictable monthly costs and ongoing support.


Members receive:

  • Same‑day or next‑day appointments
  • Secure messaging for quick questions
  • Discounts on injections, soft‑tissue therapy, and wellness services
  • Support for digestive health, metabolic health, weight management, and musculoskeletal concerns
  • Telehealth visits included each month


Non‑members are always welcome and can book any service at standard pricing.

Pricing

Clear, simple pricing helps you plan ahead.


Members

  • Monthly membership fee
  • Discounted pricing on add‑on services
  • No deposit required for appointments


Non‑Members

  • Standard pricing for all services
  • A 50% deposit is collected at the time of booking to hold your appointment
  • Remaining balance is paid at your visit


This deposit helps keep the schedule running smoothly and ensures availability for patients who need timely care.

Medicare Waiver (Required for All Medicare Patients)

Because this practice is opted out of Medicare, all Medicare patients—members and non‑members—must sign a one‑time Medicare Private Contract (waiver) before receiving care.


This form explains:

  • We do not bill Medicare
  • You agree not to submit claims to Medicare for services here
  • You may still use Medicare normally everywhere else


The waiver is signed once and kept on file. It will be sent to you electronically as part of your intake paperwork.

Scheduling

You can schedule online anytime. The system will guide you through:

  1. Choosing your visit type
  2. Completing your intake forms
  3. Collecting your credit card to keep securely on file (i.e., paying your deposits, paying for other fees, etc.)
  4. Receiving appointment reminders.


If you prefer to schedule by phone or need help choosing a visit type, you’re welcome to call.
