Getting Started Is Simple

Step 1 — Schedule & Pay for Your Visit

Choose a time that works for you and complete payment at the time of booking.
This secures your appointment and reserves dedicated time for your care.

Step 2 — Complete Your Intake Forms

After scheduling, you will receive your intake forms.

Please complete all required forms at least 24 hours before your appointment.
Be prepared to include your list of medications and supplements to make the most of your upcoming visit.

If forms are not completed at least 24 hours prior, your visit may need to be cancelled or rescheduled.

Step 3 — Prepare for Your Visit

To make the most of your time:

Ensure you have a quiet, private space for your telehealth visit
Be ready to discuss your symptoms, goals, and concerns
Test your device (phone, tablet, or computer) for video access
